Our story

From coaching client to CEO.

Dr. Mark Sanna is a second-generation chiropractor who graduated from New York Chiropractic College in 1983 and went on to build a successful chiropractic practice specializing in active care and rehabilitation. His experience as a practicing chiropractor gave him firsthand insight into both the rewards and challenges of owning and growing a healthcare practice.

Mark’s relationship with Breakthrough Coaching began from the other side of the table—as a coaching client. After experiencing the impact that coaching had on his own practice, he became increasingly involved with the company and, three years later, purchased Breakthrough Coaching. Since 1998, Breakthrough Coaching has helped thousands of chiropractors and healthcare professionals build stronger, more profitable, better-organized practices.

Today, Mark serves as CEO of Breakthrough Coaching, working with chiropractic and multidisciplinary practices across the country. Over more than 25 years of coaching, he has helped thousands of doctors and practice teams improve their leadership, systems, profitability, and quality of life—while never losing sight of why they went into healthcare in the first place.

Frequently asked questions

What could be different in my practice if I had the right coach?

Imagine walking into your practice knowing your numbers, having a team that understands what is expected of them, seeing consistent growth, making decisions with confidence, and no longer feeling like everything depends on you.

That's what coaching should help you create.

Your goals may be more patients, greater profitability, a stronger team, fewer hours, another location, an associate, or simply enjoying your practice again.

We start with one question:

What would you like your practice—and your life—to look like a year from now?

Then we help you get there.

Can coaching really help me make more money without simply seeing more patients?

Yes.

More patients aren't always the answer.

You may already have significant untapped profitability inside your existing practice. Improving collections, case acceptance, retention, scheduling, provider productivity, staffing, overhead, reimbursement, and systems can dramatically change your bottom line without requiring you to work longer hours.

The goal isn't simply to make your practice busier.

It's to make your practice more profitable, efficient, and rewarding.

Can you help me stop feeling like everything in the practice depends on me?

Yes—and this is one of the most important transformations we can help you make.

If every question, decision, employee problem, patient issue, and operational detail eventually lands on your desk, growth will always be limited by your personal capacity.

We help you develop systems, accountability, delegation, and leaders so that other people can successfully carry responsibility.

Imagine taking a vacation without constantly checking your phone.

Or leaving the office knowing things will continue to run properly tomorrow without you.

That's the kind of practice we're trying to build.

Can you help me build a team that takes ownership instead of waiting for me to tell them what to do?

Absolutely.

Imagine your team coming to you with solutions instead of problems.

Imagine meetings where everyone knows the numbers, understands the goals, and knows exactly what they are responsible for accomplishing.

That's what strong leadership and accountability can create.

We help you establish expectations, improve communication, develop leaders, measure performance, and build a culture where your people understand what winning looks like.

Can you help me finally understand where all the money is going?

Yes.

A doctor can have a busy practice, collect a lot of money, and still wonder at the end of the month:

"Where did it all go?"

We help you understand the numbers that actually drive your business so you can identify what's profitable, what's costing you money, where opportunities are being missed, and what needs to change.

The goal is for you to stop making important decisions based on feelings.

You'll have the numbers to make them with confidence.

Can you help me grow without sacrificing more of my life?

That's exactly the kind of growth we want.

Growth shouldn't automatically mean seeing patients five days a week, staying later, managing more problems, and taking your laptop on vacation.

We help you build the people, systems, capacity, and financial strength necessary for the practice to grow without requiring proportionately more of you.

A bigger practice isn't necessarily a better practice.

A better practice produces greater results while giving you greater control over your time.

Can you help me attract more of the right patients consistently?

Yes.

Imagine not beginning every month wondering where the next group of new patients will come from.

We help you create a more consistent approach to growth through patient referrals, community relationships, professional referrals, reactivation, digital marketing, social media, patient education, events, and other strategies appropriate for your practice.

The objective isn't one successful promotion.

It's creating a predictable system that keeps introducing new people to your practice.

Can you help me hire an associate without making an expensive mistake?

Yes.

Hiring the right associate can give you tremendous capacity and freedom.

Hiring the wrong one—or creating the wrong compensation plan—can become one of the most expensive decisions you'll make.

We help you determine whether you're ready, what the associate needs to produce, what you can afford to pay, how incentives should work, and what success should look like.

The goal isn't simply to put another doctor in the building.

It's to create a position that benefits the associate, the practice, your patients, and you.

Can you help me turn my practice into a business instead of another job?

Yes.

Many chiropractors created a practice because they wanted independence—only to discover that they built a business that can't function without them.

We want to change that.

Through stronger systems, leadership, financial management, delegation, and accountability, we help you build an organization that becomes increasingly less dependent upon you personally.

That gives you choices.

You can grow. Add providers. Reduce your clinical hours. Take more time away. Prepare for eventual succession or sale.

The ultimate goal is freedom—the ability to choose how you want to practice and live.

I'm overwhelmed now. Won't coaching just give me more things to do?

It shouldn't.

The last thing an overwhelmed chiropractor needs is another 47-item to-do list.

Good coaching helps you determine what matters most.

What should you do personally? What should someone else do? What needs a system? What should be automated? What should simply stop?

Instead of adding more to your plate, we help you decide what deserves to remain on it.

The goal is progress without overwhelm.

My practice is already successful. What could coaching do for me?

Success creates its own set of challenges.

You may be asking different questions now:

  • How do I become less clinically dependent?
  • How do I develop my leadership team?
  • Should I add another provider or location?
  • How do I improve profitability?
  • How do I integrate new services?
  • How do I make the business more valuable?
  • How do I eventually step away?

At this stage, coaching isn't necessarily about fixing something that's broken.

It's about helping you turn what you've already built into what it could become.

My practice is struggling. Can you help me turn it around?

That's often when having an experienced outside perspective matters most.

We start with reality—not judgment.

We look at your numbers, expenses, collections, staffing, patient flow, productivity, marketing, systems, and immediate pressures. Then we identify what can change now and what requires a longer-term solution.

You don't need 20 new initiatives.

You need to know what to do first.

That's where we begin.

How quickly could I start seeing a difference?

That depends upon your starting point and what we're trying to accomplish.

Some changes can improve performance relatively quickly. Building a stronger leadership team, changing culture, adding providers, restructuring a practice, or creating a business that operates independently of you naturally takes longer.

But you shouldn't have to wait a year to know whether you're moving in the right direction.

We establish measurable goals and track progress so you can see what's changing.

You should be able to see the distance between where you started and where you're going.

Is Breakthrough Coaching worth the investment?

That's ultimately a question only you can answer.

But don't evaluate coaching based solely on what it costs.

Consider what the improvement could be worth.

  • What would a more profitable practice be worth?
  • What would one avoided hiring mistake be worth?
  • What would better decisions be worth?
  • What would having the right team be worth?
  • What would another day away from the office be worth?
  • What would it mean to finally have a practice that doesn't depend entirely upon you?

Then compare that future to the investment required to create it.

The value of coaching isn't the coaching itself. It's what changes because of it.

How do I know if Breakthrough Coaching is right for me?

Don't start by asking whether you need a coach.

Ask yourself something bigger:

  • What do I want my practice and my life to look like 12 months from now?
  • More profitable?
  • Less stressful?
  • A stronger team?
  • More patients?
  • Better systems?
  • Fewer clinical hours?
  • Another provider or location?
  • More time with your family?
  • A practice that's becoming valuable and transferable instead of completely dependent upon you?

Now ask yourself:

If nothing changes, where will I be 12 months from now?

If there's a meaningful gap between those two answers, that's where coaching begins.

You already know where you are.

Let's talk about where you want to go next.
